Attendance Policies

PROCEDURE FOR REPORTING AN ABSENCE, LATE ARRIVAL OR EARLY DISMISSAL at Main Elementary

We understand there are circumstances when your student will have to miss school, but please follow our Attendance Procedures to ensure that you are complying with House Bill 410 to have your student properly excused. Please account for your student every day by emailing or calling the office to notify us of any absences, late arrivals, or early dismissals.

1.  TO REPORT AN ABSENCE

  • To report an absence via phone dial (937) 429-7588 and hit option 1.  

  • You may submit an absence, via email to mainattendance@gocreek.org. Include your child’s first & last name, their teacher’s name, and the reason for the absence.  Once your request has been processed you will get a return email.

2.  TO REPORT A LATE ARRIVAL

  • To report a late arrival via phone dial (937) 429-7588 and hit option 1.  

  • You may submit a late arrival, via email to mainattendance@gocreek.org. Include your child’s first & last name, their teacher’s name, and the reason for the absence.  Once your request has been processed you will get a return email.

  • You will need to come in with your child and sign them into school if arriving late.

  • If your child is arriving late due to a doctor's appointment, please be sure to bring a signed excuse from the doctor.  

3. TO REPORT AN EARLY DISMISSAL 

  • Send a signed note with your child's first and last name, teacher name, reason, date and time of the early dismissal.  

  • You may submit an early dismissal request via email to your child’s teacher and to the office at mainattendance@gocreek.org. Once your request has been processed you will get a return email.

  • Your child will be called to the office when you arrive to sign them out.

4.  EXTENDED ABSENCE/VACATION 

  • If a vacation or out-of-town trip is planned within the school year resulting in student absence, parent/guardian must submit a Vacation Request Form.  This form must be submitted 5 school days prior to the first day of vacation. Principals may approve 5 days of vacation as excused absences (will count as 5 of 10 allowable parent notes).  

Parent/Guardian may submit 10 parent notes* documenting an excused absence reason (i.e. illness, medical appointments, funerals up to three days, pre-approved out-of-state travel up to 5 days, observation of a religious holiday, etc.) After the 10 notes are exhausted, medical notes will be required for absences to be excused per House Bill 410

*A Parent Note =

  • A voicemail on our Attendance line (937-429-7588 option 1)

  • An email to mainattendance@gocreek.org

  • A phone call (937-429-7588)

  • An actual note (send in with the child)

All Medical notes must be received within 3 school days after the student returns to school from the medical appointment (a medical appointment is still EXCUSABLE with one of your 10 parent notes, but won't be medically excused without an actual note from the medical establishment).
